Your Privacy / GDPR Policy

Personal Information will remain confidential and will be stored and accessed in line with The General Data Protection Regulations (GDPR). (ICO. March 2018).

What Information Do We Need?

Basic information needed to provide a safe service for clients engraining in holistic therapies is:

  1. Client’s name.

  2. Client’s telephone phone number.

  3. Client’s address and postcode and email address.

  4. Client’s age

  5. Client’s medical / health history and or current issues experienced.

  6. When providing a service within a voluntary setting or insurance purposes and to facilitate a safe risk assessment, people attending will need to:

    • Agree to complete and sign the Initial Contact Assessment From.

    • Provide basic data such as full name or first name and initials of family name, date of birth and contact telephone numbers / email details

 

Why Do We Need It and How Will We Use It? 

As a professional therapist CV Holistic Therapies adheres to all relevant professional guidelines. Primarily we need all the details above in order to ensure that it is safe for us to provide the treatments you may ask us to provide. The information also helps with our clinical assessment and to support and recommendations we make to support client wellbeing. Having access to the above information helps to make sure we meet professional practice guidelines and also meets liability insurance requirements. 

 

Our lawful basis for processing this personal information is ‘contract’, this means that we have a legal reason to ask you for your data because we need it for contractual reasons. You do not have to share your personal data with us, but if you do not we cannot offer you a treatment with CV Holistic Therapies. You must fulfil your side of the contract (share your personal information) in order for us to fulfil ours (carry out treatment).

 

How Long Will We Retain It?

  1. We will store your information for 7 years after the date of your treatment.

  2.  Or for 7 years after you have reached the age of 18 if you are/were under 18 when your treatment took place. 

 

Will We Share Your Information?

Your confidentiality is paramount for us to work with you in a safe, honest and trusting way. We will not share your information with anyone outside of CV Holistic Therapies, unless required to do so in law. If a client requests information to be shared with another health professional written con sent will be needed.

 

Consent

It is requirement to provide informed consent for all therapies. In the event that this can not be provided treatment will be declined.

GDPR Guidelines state Clients have the following rights:

  1. The right to be informed – this is how your information is stored and used.

  2. The right of access – you can see the records held about you.

  3. The right to rectification – you can as us to make changes to your records if it is incorrect or incomplete.

  4. The right to erasure / the right to be forgotten – you can ask for your records to be erased – this may not be possible due to legal reasons.

  5. The right to restrict processing – you can request limits on how your information is held and processed.

  6. The right to data portability – under some circumstances you can request your data to be copied and shared with other professionals.

  7. The right to object – you can request that some of your information is not used or that it’s used only for certain purposes.

  8. The right not to be subject to automated decision-making including profiling (not used by CV Holistic Therapies).

Client Contact and Marketing

CV Holistic Therapies will contact Clients under the following circumstances:

  1. In response to a text message or the completion of the contact form via the website.

  2. Following requests from social media sites.

  3. To remind clients of appointments.

  4. To check on client’s wellbeing following appointments where appropriate.

  5. Clients may be contacted by email or text messaging with up-to-date marketing information, provided signed consent has been provided. This consent may be withdrawn at any time by emailing us at cvholistictherpaies@gmail.com or calling us on 07961327075

 

Complaints

As an open and transparent service is someone feels information has been handled in the wrong way or not in accordance with GDPR Guidelines.
